374 MDG: Optometry Clinic Services (3rd Floor, Main Hospital)

Glasses prescription update - Active duty members are eligible to get 1 pair of frame of choice per year and 5A (known as BCG) glasses as needed.  Gas mask inserts and ballistic eye ware inserts are issued.  Glasses prescription expires after 1 year.

Limited Contact lens prescription update   - For Non flyers, patients need to bring contact lenses and contact lens information such as boxes and/or written prescription to the exam.  Initial Contact lens fit service is only for flyers.

Renewing state side driver's license - If your eye exam in Optometry clinic is within a year, appointment is not required.  Stop by on our normal duty hours with your form.

Color Vision Test for Flight line Driver's License - Appointment not required.  Walk in on normal duty hours.

Copy of glasses/contact lenses prescription - If last eye exam is within one year, appointment is not required.  Walk in on normal duty hours with Military/dependent ID.   If last eye exam is more than one year or close to one year, please make an appointment at DSN 225-8864.

Refractive eye surgery - Must be over 21 years old, minimum 6 months retainability in Yokota after Surgery and not scheduled to have any TDY or Deployments until 4 months after surgery.

Glasses/inserts order status - Please send us an email to check on your order status.  Normally it takes about four weeks and may take longer depends on your prescription.   

Yellow on IMR? - Yellow on your IMR for medical equipment means gas mask insert is on order.  It will turn green automatically once we receive the gas mask insert and scan in.  If it turns red, please send us an email and we will check the order status.

Monday and Wednesday: 7:30 a.m. to 4 p.m.

Tuesday and Thursday: 7:30 a.m. to 3:30 p.m.

Friday: 7:30 a.m. to 12 p.m.
