Yokota's #1 mission is readiness...are you medically ready? Published April 26, 2012 By Airman 1st Class John D. Partlow 374th Airlift Wing Public Affairs YOKOTA AIR BASE, Japan -- Checking medical readiness is now just one click away and allows active duty members to go directly to their own web page. "This Air Force link is available 24/7 from a CAC enabled computer. Airmen can see if they're 'Green' or medically ready to re-train, go TDY, deploy, PCS, separate or retire," said Maj. Christine Cornish, 374th Medical Group public health flight commander. "Airmen can also print out their Mobility/Duty Restriction AF Form 469 or PT Test Exemption AF Form 422 once they're approved. The web-site is also useful to obtain a dependent's (under 18 years old) shot record information...so there's no need to stop by the clinic to get shot records for the CDC or school. Just go to your MyIMR web page." Approximately 2 percent of the 374th Airlift Wing personnel are "Red"=overdue and another 15 percent are "Yellow"=due. "Look at the information block below your web page MyIMR info. It shows instructions and phone numbers to help complete medical requirements," said Cornish. "And if additional services are needed, we encourage you to call or e-mail the contacts shown for more assistance." To check your IMR status, visit MyIMR at https://asims.afms.mil/imr/MyIMR.aspx.
